1. INTRODUCTION
1.1. The Mounjaro CareConnect Patient Education Program Singapore (“Program”) is initiated and managed by DKSH Singapore Pte. Ltd. (“DKSH”). The Program is available only to patients who have been prescribed Mounjaro® KwikPen (tirzepatide) by their treating physician for approved therapeutic indications in Singapore, as per local regulatory approvals.
Indications:
1.2. The Program aims to provide self-serve and on-demand patient support via the Connect Patient platform, delivering educational resources, treatment reminders, and patient engagement services to help you manage your therapy effectively.
1.3. Participation in this Program is voluntary. You may withdraw from this Program at any time upon request without any prejudice to your healthcare and/or benefits.
1.4. Conflict of Interest Disclosure
In the event you are a participating healthcare practitioner or a family member of the participating healthcare practitioner who is engaged in the program or you are a staff or family member of DKSH, you shall disclose such conflict of interest prior to such enrolment. You will be required to complete the declaration/disclosure form by clicking the below link.

You are required to provide information that is true, genuine, and accurate in the declaration/disclosure form. You are aware DKSH shall have the absolute right and discretion to reject application of a patient into the program when conflict of interest exists.

2. PROGRAM DETAILS
2.1. This Program is an exclusive service offered by DKSH for doctors to enhance care for patients who have been prescribed Mounjaro® (tirzepatide) for approved indications. Under this Program, eligible patients will receive access to the Mounjaro CareConnect Patient Education Program via the Connect Patient platform, which provides educational resources, treatment reminders, and optional self-monitoring tools to support adherence and improve treatment outcomes. Patients can enrol using a QR code and unique enrolment code provided by their treating doctor. Only patients who have successfully completed the enrolment process and met the Program criteria will be eligible to access Program benefits and services.

By joining this Program, you agree to the terms in this Consent Form. 3.2. Program start date: 26 Dec 2025 3.3. This Program is not a substitute for physician consultations. DKSH will not provide diagnosis or disease management advice; all clinical decisions remain with your treating physician. 3.4. You must keep your personal details up to date in the Connect Patient portal . 3.5. You may withdraw at any time by contacting DKSH at mounjarocare@dksh.com, or through your treating physician.
4. OTHER TERMS:
4.1. Notwithstanding the validity of this Program as mentioned in Clause 3.2 above, DKSH may amend and/or terminate all or any part of this Program any time provided always that a one (1) months’ written prior notice shall be given. This clause shall not, in any way, affect the rights and benefits of any patient who enrolled in the Program prior to the said amendment and/or termination. To avoid any confusion, the patient who enrolled into this Program will continue to enjoy his/her rights and benefits until the patient decided to discontinue or until the patient is no longer eligible in this Program. 4.2. All decisions by DKSH with respect to this Program are final and binding. 4.3. DKSH reserve the right to disqualify you from participating in this Program in case of any misuse of this Program and/or if you breach any terms and conditions contained herein. 4.4. You agree that this Program does not take effect and is not intended to take effect as an incentive or reward for past, present or future willingness to purchase any DKSH product or as an incentive to grant an interview for any sales or marketing purpose.
5. PHARMACOVIGILANCE STATEMENT
5.1. You should immediately report any health concerns, adverse events or product technical complaints to your treating physician. You shall refer to your treating physician for any questions regarding DKSH products.

All components of the Terms and Conditions and any service level agreements shall be read together and construed, to the fullest extent possible, to be in concert with each other. DKSH provides the Platform, Site, and Services to assist patients prescribed Mounjaro® (tirzepatide) for approved indications in Singapore. Our Platform, Site, and Services require your active engagement and participation and you understand that, despite your efforts, individual user’s results will vary for a variety of reasons. We may also use third parties, in whole or in part, to provide the products and our Services.
To access this Platform, you must have been invited by a Healthcare professional to join the Platform and have created a Platform account, and provide accurate personal and contactinformation and documentation for which you have a legal right to copy, publish, share, store, or otherwise use (whether relating to yourself, or to another individual under your care, custody, and control, as the case may be)** (“User Data”)**, that is protected by a username and password (your “Account”). You represent and warrant that you are 18 years old or older. You agree to provide DKSH with accurate and complete Account information when you register for and at the Platform, and that you have sole responsibility for the accuracy, quality, integrity, legality, reliability, and appropriateness of all data you provide. If DKSH believes the information you provide is not correct, current, or complete, DKSH has the right to refuse your access to this Platform or any of its resources, and to terminate or suspend your access at any time. You agree to keep your password and other Account details secret, and not share them with anyone else, in order to prevent unauthorized access to your Account. If your contact information or other Account information changes, you must update your Account details promptly. After you create an Account, DKSH and/or its authorized third party may use and access the User Data as necessary for the management and administration of the Platform.
If you access the Platform through a legal person such as your business partner, your employer, or through an authorized DKSH partner, you acknowledge and agree that DKSH and/or its authorized third party may have been designated to access, control, and manage your Account and your User Data. You, not DKSH, are solely responsible for your Account and User Data. DKSH is not liable for any loss or damage arising from any access to, sharing or use of, your Account and User Data. You must inform DKSH immediately at mounjarocare@dksh.com if you believe there has been unauthorized access to your Account or your User Data.
You may use this Platform for purposes expressly permitted by this Platform, as follows:
You may not use this Platform for any other purpose, including any commercial purpose not expressly permitted by written agreements with DKSH, without DKSH’s express prior written consent. For example, you may not (and may not authorize any other party to) (i) co-brand this Platform, or (ii) frame this Platform, or (iii) hyperlink to this Platform, without the express prior written permission of an authorized representative of DKSH. These Terms and Conditions will be governed and interpreted pursuant to the laws of Singapore, notwithstanding any principles of conflicts of law. You specifically consent to personal jurisdiction in Singapore in connection with any dispute between you and DKSH arising out of these Terms and Conditions or pertaining to the subject matter hereof. The parties to these Terms and Conditions each agree that the exclusive venue for any dispute between the parties arising out of these Terms and Conditions or pertaining to the subject matter of these Terms and Conditions will be in the courts in Singapore. If any part of these Terms and Conditions is unlawful, void, or unenforceable, that part will be deemed severed and will not affect the validity and enforceability of any remaining provisions. These Terms and Conditions constitute the entire agreement among the parties relating to this subject matter. Notwithstanding the foregoing, any additional terms and conditions on this Platform will govern the items to which they pertain. DKSH reserves the right to revise or modify these Terms and Conditions at any time by updating this posting.
